This is a raw dataset with neighborhood profile data developed by DataHaven using the 2020 5-year American Community Survey, as well as other public statistics calculated by DataHaven for individual Hartford and West Hartford neighborhoods.

The North Hartford Triple Aim Collaborative, the newest Wellville (as of October 2016), spent 2017 focused on establishing a strong, inclusive collaborative and investing in community-based efforts to address the social determinants of health.
