This Community Health Needs Assessment was carried out in 2013 to identify current and future healthcare needs in the community and to improve and strengthen programs and services provided to address them. They surveyed residents in the service area and local community agencies, and compiled this report which addresses areas of interest, including cancer, diabetes, and prenatal care.

Milford Hospital led a comprehensive Community Health Needs Assessment (CHNA) to evaluate the health needs of individuals living in Milford, Connecticut beginning in 2013. The assessment was conducted in a timeline to comply with requirements set forth in the Affordable Care Act, as well as to further the hospital’s commitment to community health and population health management.
